It doesn't start with soldiers "fragging" their own officers (as in Vietnam). It starts with people dragging their feet and doing a bad job at implementing orders.
Remember Trump's military parade?
They drug their feet. They deliberately made the parade comical. That parade should have been a warning sign to Trump that he was in danger of losing command of the military.

One of the most important rules in the military is to never give orders that won't be followed.
Yes, militaries punish people for not following orders for precisely this reason, but even so, giving orders that flat-out won't be followed undermines your own leadership in dangerous ways.
