I figured out how to create custom hiking maps that I can view on my Apple Watch (and any other device/app that can view GeoTiff maps). I used the QGIS desktop app to turn a regular PDF into a GeoTiff, and the Topo Maps+ app on the watch/phone to view it. Here's a guide, with an example of the Middlesex Fells trail map.
