We've just finished teaching Austere Emergency Care (AEC), developed by Specialized Medical Standards, in and around our Maltese base.

AEC is a mix of online material, classroom lectures, skills stations and scenario based missions, all focused on developing a medic's prolonged field care skills and knowledge.

Every student's growth, in terms of skills and confidence, was evident as the course progressed. Teamwork in the day long final training scenario was excellent.

The College is running an ACLS course at the Kilimanjaro Christian Medical Center (KCMC). Aebhric O'Kelly is monitoring the course and is concurrently running an ACLS instructor course. It is important when setting up a training system to also set up the ability for self sustainment.

Holy shit, if this pans out that would be *amazing*.

The tldw is that when mosquitos take a blood meal from someone on ivermectin, ~50% of the mosquitoes just... die within 2 days, and none of the mosquitos laid eggs. There's also some (conflicting) indications that it inhibits plasmodium reproduction. (y'know, the parasite that causes malaria) Plus ivermectin's cheap and generally safe.

Current modeling says 3 doses over 3 months could reduce new malaria cases by 71%.

Guerilla Casualty Care on the Future Battlefield

#Guerrilla #casualty care is modelled after guerrilla warfare principles: decentralised, mobile, hidden, and resilient.

The concept aims to provide #trauma care in degraded environments, especially where #evacuation and surgical support are not guaranteed.

The doctrine moves away from reliance on the “Golden Hour” and towards prolonged, distributed care across web networks of casualty care nodes.

Guerilla Casualty Care on the Future Battlefield | College of Remote & Offshore Medicine Foundation

The article by Brown et al. (2025) introduces the concept of guerrilla casualty care nodes embedded within web networks, designed to deliver trauma care in contested, irregular warfare environments where traditional evacuation and surgical support are not available. Main Concepts The use of CATTs Clinical Capabilities of CATTs Tactical and Operational Requirements Global Applications For medics seeking relevance in future operations, the article offers a clarion call: training and doctrine must evolve. The traditional ‘golden hour’ is increasingly untenable, and patients may require care over extended periods in austere settings with limited support. Adoption of a distributed network model and resilience mindset ensures continuity of care even when evacuation or surgical assets are delayed or incapacitated. In sum, the insights presented are foundational for any remote paramedic preparing to operate at the cutting edge of trauma medicine in contested environments. The Bottom Line: Remote Medics need to learn and adopt the guerrilla model of healthcare.
