Changes:
• Improvements to several figures.
• Various small improvements to the text.
• Several minor corrections, including a number of reader-submitted ones.
• All end-of-line word hyphenations now follow the Oxford University Press standard. (The TeX/LaTeX #hyphenation algorithm does a good job, but is not perfect.)
• Many previously-bad page breaks have been adjusted.
