If you make your protagonist perfect and powerful, then you really have to bang them up a lot to keep the stakes high. If it's fantasy, then you can use magic to heal, then they can keep breaking limbs or getting concussions, or whatever. Science fiction, maybe there's advanced healing so concussions are safer? But what do non-genre fiction writers do?
