ECMO and awake ECMO have been a game changer for critical patients on the waiting list for a lung transplant. During the COVID-19 pandemic a lot of patients went on ECMO. When carefully selected the outcome was very good. However, the numbers of potential ARDS patients and ECMO machines did not always match leading to longer distance ICU transports to get access to an available device.

Preserving and conditioning organs for lung transplant has evolved quite a lot in the past decade.
The possibility to use reconditioned perfused organs or even transport lungs using a mobile device is available and used by selected transplant centers increasing the acceptance rate of organs.
I expect to see more use of both techniques in the coming years.

@gkd5mao @NewYorker We are blessed with clinicians like Jon and "blessed" by being a high endemicity area for Hantaviruses, which means we have always had ECMOs (since the large outbreak in the early 90s). It is a game changer and the piece does the field and pioneers like Jon justice
