Government agencies and insurers have touted the importance of #primarycare and how they will fix it throughout my career yet it has continued to get worse. Prepaying for routine care via them does too much harm.

I stopped waiting for them nine years ago. #Physicians and patients can fix most of what's wrong with primary care without them.

I'm paid by the month now. Works well.
I own my own practice but I would want a union if I worked at the #toobigtoofail #hospital network.

I have a weird perspective with a career that started in academic family medicine, went into health computing industry for 20y, then ended up as an administrator with a small practice.

The biggest problem I saw in primary care over that time was the growth of uncompensated paper work and especially EHR work. Often late at night from home.

Paying providers hourly would eliminate that abuse in months. Time would be used far more efficiently in patient care.
