Echo 11 meningitis in neonates

We received an ECHO 11 meningitis alert from the National Reference Center today. has anyone seen cases? there is a high incidence of affected male twins. Is that coincidence or a low number of cases? I do not understand that. Any ideas?

@99nicu Thanks @KMasjosthusmann for high-lighting this on Tw (and sorry for being slow on the ball to promote your post)

“The WHO confirmed at least 26 infants in Croatia, France, Italy, Spain, Sweden & the UK had been infected with a rare type of enterovirus, called echovirus-11. Eight of those babies died, with most deaths in France following organ failure & sepsis.”

@stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez I answered with a link to your post on this at the forum, have you seen any of these cases?
@Gotped @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez we’ve had one enteroviral meningitis recently but didn’t sero-type.
@nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez Since the mortality rate is rather high, we should really interact more as a neonatal society about it. Possible treatment options IVIG, Capsid inhibitors etc.
@nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez The antiviral drug was an suggestion from UpToDate in treating enteroviral infections, referring to this study https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4765488/
@nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez #NeoIPC Do you have thoughts on antiviral drugs @brar ?
@Gotped @nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez
The trial cited above looks promising although this definitely still is trial land because of the remaining questions (see their inclusion criteria).
Then again any new trial would need to respect the existing results which makes it borderline infeasible.
Note that the two surviving infants in https://www.eurosurveillance.org/content/10.2807/1560-7917.ES.2023.28.22.2300253 also received antiviral drugs (Pocapavir).
Eurosurveillance | Severe and fatal neonatal infections linked to a new variant of echovirus 11, France, July 2022 to April 2023

We report nine severe neonatal infections caused by a new variant of echovirus 11. All were male, eight were twins. At illness onset, they were 3–5 days-old and had severe sepsis and liver failure. This new variant, detected in France since April 2022, is still circulating and has caused more fatal neonatal enterovirus infections in 2022 and 2023 (8/496; 1.6%, seven associated with echovirus 11) compared with 2016 to 2021 (7/1,774; 0.4%). National and international alerts are warranted.

@brar @nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez Also a capsid-inhibitor! I would say that we really need infection specialists help with these different antiviral drugs. Or some official recommendations 🫤
@Gotped @nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez That's exactly what I'm trying to initiate and support. Many infection specialists don't even dare to think about treating a patient weighing 980g and many neonatologists leave their comfort zone when infections/pathogens get complicated. There's so much to learn from each other and together.
@Gotped @nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez maybe @vmcorman wants to share his thoughts on antivirals to treat echoviruses.
There is nobody in the fediverse whom I'd trust more when it comes to virology.
@brar @nrao @stefanjohansson @99nicu @KMasjosthusmann @jramonfernandez @vmcorman Great suggestion! Welcome to our Nicuverse @vmcorman , nice to have you visiting!
