@thickurt not sure, but I think that's the first blossom of that plant. In German called 'Königsblüte' (king's blosssom), dunno what that's in english.

We nip these off because the plant would invest most of it's energy in this one tomatoe, resulting in less overall produce from the plant during the season. It's done regularly with peppers but many gardeners do this also with tomatoes.

...or, it's just a heirloom sort where these kind of blossoms are normal. *shrugs*
