Emerging evidence suggests an association between concussion and increased risk of suicide, suicide attempts, and suicidal ideation. Concussion affects approximately 2 million youth each year, costing over $1 billion in medical expenses annually. Prior studies indicate that individuals diagnosed with concussion have twice the increased suicide risk compared to those without. Furthermore, recent Youth Risk Behavior Surveillance Survey findings show significant associations between sports-related concussion in the past 12-months and suicidal ideation and attempts. Adding to the literature on this topic is a new retrospective longitudinal cohort study of over 417,500 youth and young adults aged 5–24 diagnosed with either concussion or orthopedic injury and followed for up to 10 years. The authors of this new study provide some possible mechanisms that may explain this association between suicide and concussion: 🔸 concussions can disrupt brain function, 🔸 post-concussion symptoms can lead to psychosocial stressors, such as an inability to participate in sports, social activities, or school, which may cause isolation and emotional distress, 🔸 cognitive deficits after concussion may impair judgment and decision-making, potentially leading to impulsive behavior, 🔸 concussions may worsen pre-existing psychiatric and psychosocial disorders, which are known suicide risk factors. Despite these risks, the overall 5-year suicide risk for youth with concussion remains low. The authors further note that their findings in this new study have several clinical implications: 🔹 concussion underreporting due to misconceptions might bias suicide risk estimates by undercounting concussion and post-concussion suicide cases, 🔹 youth and caregivers may assume that symptoms resolve quickly or go undetected on imaging, leading to inadequate follow-up, 🔹 clinicians may not routinely consider concussion history during mental health assessments. The study’s findings underscore the importance of identifying youth at elevated risk and implementing targeted prevention strategies. Such strategies may include early screening for suicide risk and mental health concerns, as well as integrated, collaborative care models to support recovery and reduce long-term risk - particularly for youth with repeated concussions.
