What were the old collation rules for the GPO telephone directory?

They used to be printed at the front, or possibly at the back.

I have this vague memory that there were special rules for "van", "le", "de", "von", "ap", and suchlike where one had to initially ignore these. And there was a rule about hyphenation, too.
