A car is a multiple ton metal box that typically moves at speeds far exceeding the reaction speeds of humans. In the US, "male" dummies are used and no other dummies are required. So if you don't match the "average male" body used during testing, you are automatically at higher risk. Women, for example, are 20-30% more likely to be killed per crash (normalized for crash count, since men tend to get into more crashes). Children and car seats have historically been entirely omitted from crash testing.
And that assumes you're inside the vehicle. Cars are only crash tested against the same class of vehicle, so big vehicles crushing little ones isn't even considered. There's no safe way to be a pedestrian around multiple ton metal boxes.
Humans are not good at driving, and sleep deprivation makes this much worse. 17 hours awake is roughly equivalent to a 0.05% BAC, and 24 hours is equivalent .1%. A BAC of 0.1 is over the legal limit basically everywhere. You know who doesn't sleep? Parents of young kids.
Even if you are 100% careful and defensive, you are surrounded by people who can't be. If your vehicle isn't a murder box, then you are vulnerable to one.
This is the worst possible system. Don't let anyone pretend that there is ever a way for it to be "safe."
