What are people using as alternatives to google forms?

My main two use cases are capturing the addresses of new magazine subscribers (ongoing) and running my annual reader survey. I need to be able to embed the survey as html, and then to have the responses in a sheet/csv format.

There are then ad hoc other things, but nothing is hugely complex in terms of the captured data requirements.

I'd obviously love something free, but open to some kind of payment if there's a suitable model to account for my pattern of usage.
