This morning we had a report of an important system not working properly.
Tracking back to yesterday afternoon, a colleague made a change to bring that system back into compliance with our managed config (this happens all the time) but that broke it.
In our whole-dept Incidents chat where it was being discussed, they explained what they did, why, and accepted the responsibility.
An exemplar to some colleagues who always obfuscate their responsibilities!
