@thisismissem @sgf "blame" is not the same thing as "assigning responsibility".
A good red flag for this is teams that say "We do #blameless #postmortems by not naming anyone in the postmortem".
No! You know you have a blameless postmortem culture when you *can* name people in postmortems without it causing problems.
This can be exceptionally hard to achieve, but it's worth it.

A post-incident review had phrases like "an engineering mistake caused ..." and "the offending PR was reverted".
I suggested removing the judgemental language and sticking to the plain facts.

"I would never have written it that way if it was about somebody else"
Not only is the doc now much better (we got some good lessons for everyone), we're both having much better days for that small bit of feedback.
