My wife is an addiction medicine person in our area and apparently, despite an attempt at state regulation https://leg.colorado.gov/bills/sb25-072 concentrated 7-OH Kratom has made its way into circulation. Well I guess it was always there but now it's being synthesized and sold as a more concentrated supplement, there's no federal regulation since the 2018 FDA said that "the science isn't complete" and washed their hands of it.

The alkaloid 7-hydroxymitragynine (7-OH) is a naturally occurring substance in the kratom plant (Mitragyna speciosa), but only a minor constituent, described as early as 1994, when it was
reported to comprise about 1.6% of the total alkaloid content of kratom leaves (Ponglux et al.,1994). "
"In this assay, 7-OH displayed approximately 13-fold greater potency
than morphine and 46-fold greater potency than mitragynine. The inhibition was reversed by
naloxone, suggesting the effects are mediated via mu opioid receptors (Matsumoto et al., 2004)."
Except that my wife says that people are reporting less of a rescue effect from naloxone so keep it handy, use it but continue to monitor the person you find passed out on the sidewalk until EMTs arrive!
