Had a perfect stab with no pain or even discomfort. However, the moment we pulled the needle out, whooooooooooosh with the blood!

This is a really good reminder that even when you've been literally doing injections for years, you can and will sometimes get a bleeder.

It's why it's recommended to always have something on hand to stop the bleeding, as it'll quickly stop (barring you having issues with clotting or taking certain meds).

And always have your medical plaster (aka bandaid) ready to go 🩹

Per usual, used the IM method we learnt from the Plume guide. Got a perfect spot tonight: no pain going in or out, despite us being emotionally and mentally exhausted. Teeny spot o' blood, but quickly wiped and covered with a medical plaster.

No joy out of doing the injection tonight though. No euphoria. Just... nothing.

Made ourselves do it earlier (before dinner) this evening, so we'd:

• Actually do it on the correct day (rather than run out of spoons and have to do it the next day).
• Have a better chance of working what we'd been doing wrong with a few recent injections with the auto-injector.
  • We reckon our grip had been slipping whilst doing Z-track when we were tired, causing issues when pulling out the needle.

Absolutely zero issues this evening 🥰

Hey does everyone here with a penis know that suppressing T prevents automatic nightly erections and that, without regular manual erections, this can cause atrophy and loss of material/ability of material to stretch?

Even if you don't care inherently, this can be important if you plan to have a penile inversion vaginoplasty at some point. There are other options with their own pros and cons but it's something to be aware of.

The recommendation I've seen around for maintenance is to keep it erect for about ten minutes at least three times a week.

From what I'm hearing it seems like we should make a point to mention this to people more consistently.

Per previous injection post, we're keeping our estradiol dose a little lower until we've had further blood tests and investigations to determine whether the last blood test results were in error, or some other factor has affected our estradiol level.

Our gender clinic has sent a request to our GP surgery to retest, so we're planning to keep our dose lower at 4 mg weekly until we book an appointment, then inject only a very small amount the week before.

Either our estradiol level will then be back within range on the test or it'll suggest that something else is at play.

Our current best bet is our combo of ADHD meds (lisdexamfetamine and guanfacine) messing with hormone levels.

We may be able to use this to justify further testing to rule out stuff at GP though. We'll find out soonish, we guess.

I got the evaluation of my hormone levels from my endocrinologist and he writes that my estrogen level of about 50 pg/ml is in the target range. It seems kind of low to me though and I get conflicting information anywhere else, so I would like a second opinion on that. Can someone with a bit of experience tell me if this is correct?

Just for clarification: I use estrogen gel daily for transdermal application.

We didn't previously have the spoons to advise why we've temporarily dropped our normal dose down from ~7.5 mg to ~5 mg as of our previous injection on 2026-01-16.

It can be summarised as follows:

• We had had multiple blood tests between 2023 and late 2024 whilst doing injections.
  • We used to do injections on Saturdays, but switched to Friday evenings.
  • All our draws for blood tests have been done Friday mornings.
• From the test results, we determined that our estradiol level was stable and could be reliably estimated thusly:
  • Day before next injection - 166 pmol/L per 1 mg of EEn injected
  • Morning of day of next injection - 125 pmol/L per 1 mg of EEn injected
• We did not have any blood tests relating to GAHT during 2025 for multiple reasons.
• We had a wide-ranging blood test on 2026-01-09, so we injected a lower amount the week before (2026-01-02).
  • The result should have been between 500 and 600 pmol/L, based on the amount injected.
  • The actual result was more than double this (1349 pmol/L).
  • Our testosterone level was also out of whack (2.9 nmol/L!!!), despite our progesterone level being high (45.2 nmol/L).
• We're uncertain whether:
  • Other medications we're taking have affected our E2 and T levels.
  • The tests results are inaccurate.
  • Something else has affected the results.
• Until we can arrange another blood test to be sure, we're lowering our weekly dose to a mid amount.
  • Whilst we're aiming to keep E2 largely within a 750 to 1500 ish pmol/L range in the long term, we're not worried by peaks of up to about 2500 pmol/L in the short to medium term, which is why we're not lowering it more for now.
  • We also don't want to lower it too much, in case the test results were inaccurate.
