Use of second prevention drugs and its influencing factors among patients with cardiovascular diseases at ages of 35 to 75 years in Jiangsu Province

<p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d8211345e126"> <b>Objective</b> To investigate the use of second prevention drugs and its influencing factors among patients with cardiovascular diseases at ages of 35 to 75 years in Jiangsu Province, so as to provide the scientific evidence for improving cardiovascular disease control in Jiangsu Province. </p><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d8211345e131"> <b>Methods</b> Based on the data captured from Screening Program of High-risk Populations for Cardiovascular Diseases in Jiangsu Province from 2015 to 2019, a total of 3 576 patients with self-reported ischemic heart diseases and/or ischemic stroke were enrolled. Participants’ demographic features and use of second prevention drugs were collected, and the correlation between individual characteristics and use of secondary prevention drugs was examined using a multivariate mixed-effects model. </p><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d8211345e136"> <b>Results</b> Of the 3 576 participants, 23.2% used statins or antiplatelet drugs, and women [odds ratio ( <i>OR</i>) = 0.59, 95% confidential interval ( <i>CI</i>): (0.47, 0.73)], current smoking [ <i>OR</i> = 0.64, 95% <i>CI</i>: (0.49, 0.82)] and current alcohol consumption [ <i>OR</i> = 0.76, 95% <i>CI</i>: (0.58, 0.99)] resulted in a lower possibility of use of statins or antiplatelet drugs, while a higher educational level [as compared to primary school and below, junior high school: <i>OR</i> = 1.72,95% <i>CI</i>: (1.35, 2.20); senior high school: <i>OR</i> = 1.75, 95% <i>CI</i>: (1.21, 2.51); university and above : <i>OR</i> =1.94, 95% <i>CI</i>: (1.06, 3.57)] and a history of hypertension [ <i>OR</i> =1.38, 95% (1.13, 1.70)] resulted in a higher possibility of use of statins or antiplatelet drugs. </p><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d8211345e182"> <b>Conclusions</b> The use of secondary prevention drugs is not satisfactory among patients with cardiovascular diseases in Jiangsu Province. Females, patients with a low educational level, patients without a history of hypertension, current smokers and patients with current alcohol consumption should be given a high priority for cardiovascular disease control. </p><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" class="first" dir="auto" id="d8211345e188"> <b>摘要:目的</b> 评估江苏省35~75岁心血管疾病患者二级预防用药情况及影响因素, 为加强全省心血管疾病防治工作提供 科学依据。 <b>方法</b> 基于 2015—2019 年江苏省心血管病高危人群筛查项目数据, 纳人其中自报既往患有缺血性心脏病和/或 缺血性脑卒中患者, 通过问卷调查, 收集患者一般情况和用药信息, 使用多变量混合效应模型评估个体特征与二级预防用药 的相关性。 <b>结果</b> 纳人研究3 576例, 23.2% 使用他汀类或抗血小板药物;女性 ( <i>OR</i> = 0.59, 95% <i>CI</i>: 0.47 ~ 0.73)、现在吸烟 ( <i>OR</i> = 0.64, 95% <i>CI</i>: 0.49~0.82)、现在饮酒 ( <i>OR</i> = 0.76, 95% <i>CI</i>: 0.58~0.99)患者使用他汀类或抗血小板药物可能性更低;教 育水平越高(与小学及以下比较, 初中: <i>OR</i> =1.72, 95% <i>CI</i>: 1.35~2.20; 高中: <i>OR</i> =1.75, 95% <i>CI</i>: 1.21~2.51; 大学及以上: OR = 1.94, 95% <i>CI</i>: 1.06~3.57)、有高血压史 ( <i>OR</i> = 1.38, 95% <i>CI</i>: 1.13 ~ 1.70) 患者使用他汀类或抗血小板药物可能性更高。 <b>结论</b> 江苏省心血管疾病患者二级预防用药使用不太理想, 应重点关注女性、低教育水平、无高血压史、吸烟和现在饮酒患者。 </p>
