Said this before, but if anyone's looking for NVDA add-on ideas, here's a couple for you:
1. An add-on that makes the speech pause slightly longer between paragraphs when doing a read-to-end. You could have the length be configurable.
Speaking for myself, I hardly ever use read-to-end as I prefer having a longer pause between paragraphs. So I'd love to have this without needing to keep pressing ctrl+down arrow. When reading a novel for example, its not often clear to me who's saying what when the pause length for sentences and paragraphs is the same.
2. A simple EQ or tone control for speech output. If you had only one sound card with a pair of bassy speakers or headphones connected, you could then turn down the low-end of your speech synth if it has too much, without affecting everything else you're listening to.. eloquence I'm looking at you.
There's probably system-wide EQs that can do this, but having it as an add-on I think would be more easier to set up.
You could also use two sound cards, which I do, but that's not always convenient for people.
some might also find it useful to boost the mids or highs as well..
I posted these ideas to the add-ons e-mail list as well.
I'd make both myself if I knew Python, but it's beyond me.
