The overwhelmingly most likely cause of this incident was driver error, as is the case in virtually every crash.

If the media report on driving responsibly, they’d easily change the public perception of road danger. Which would pressure the government to change the law to suit vulnerable road users not drivers, and this could easily save thousands of lives. 5 people a day are killed by drivers and epidemic that the press has normalised because according to them ‘we all need to driver everywhere we want, in whatever vehicle we like, at whatever speed we want’.
