Humalog and Lyumjev are both lispro insulins. The difference is the acid used in Lyumjev to stimulate blood flow at the injection site for faster absorption.

Often, this is not noticed with pen injections. But in a pump, where there’s a relatively continuous infusion and occasional bolus infusions at the same place, Lyumjev can become painful.

Some people adjust to it. I have not.

Those who like the performance of Lyumjev but find it too uncomfortable to use in an insulin pump might try mixing Humalog and Lyumjev to reduce the bite. I know of one person that does a 1:1 ratio of the two, and another person who does 3:1 Humalog to Lyumjev (25% Lyumjev).

I am using a 3:1 mix right now. It feels like full strength Lyumjev.

My first go with Humalog was just 100% Humalog so I could assess how I respond to it. There was no pain. In fact, I didn’t feel boluses at all. I did notice the difference in timing, but it was super minor and easy to adjust to.

Even though I am now no longer planning to mix Lyumjev and Humalog, this was still a successful experiment because I learned something. And that’s good! I now have a path forward with my insulin use, and honestly, keeping it simple with something I’m pretty sure will work fine feels a bit better anyway.

One thing I’ve learned in the management of my illnesses is experimentation to find the best management is part of it.

Today I started using Humalog in my pump. Lyumjev was a bit rough.

Just did my first bolus after several hours of just basal rate. I didn’t feel it. No irritation. It was almost 9U total.

Now I just need a good reliable CGM and this will be probably as good a pump experience as I can get.

I had a mostly full Lyumjev pen when I started on my pump. Which is kinda unfortunate. Not wanting it to go to waste, I decided to see how easy it would be to fill the Mobi tank from it with the fill adapter.

Was super easy, actually!

I did run into a flaw in the cannula applicator, though. I hope my cannula isn’t bent as a result. I guess we find out soon.

First site change done!

Moved from stomach to leg to see if I like it better. I think I will.

Using Lyumjev in the pump is a tad rough. We shall see if that persists with the leg site. If it’s something that’s just going to not be fun no matter what, I might ask about Fiasp, or downgrade to a less rapid insulin.

I actually don’t know this answer yet. I go pick it up later today.

The obvious joke would be 1 vial per month, so 3 vials. So almost $2.3k for 30mL of insulin.

Made the trip back to the pharmacy. One of the previous ladies was there. Apparently she's been out for a bit and is back now.

She agreed the Lyumjev prescription was not filled correctly. So she's going to help me out.

So I should be covered!

I was in a hurry to pick up my prescriptions. Tomorrow I will be going back to ask for an explanation how this is a 90 day supply of insulin.

On the bright side, I did also get Mounjaro 5mg. So there’s that. I think I’m going to wait on restarting it so I can build up some buffer supply.

Right after posting this, I got the prescription ready notification.

And CVS has now failed me in another way.

My doc updated my script to 20U per day. A box of pens is 1500U total. Not accounting for priming, that’s 75 days, right? Well, priming eats into some of that. So less than 75 days (probably closer to 70 days).

They gave me 1 box to cover 90 days.

Meh, I’m very low on Lyumjev and CVS is taking their sweet time refilling the script. Like, they have the insulin. I confirmed it earlier today. But it’s now late enough in the day that I’m concerned I’ll not be able to get it today.

I might run out by tomorrow.

Not a fan of the new people there. Don’t know what happened to the previous staff.
