Why is #JustCulture still a struggle? Without psychological safety, reporting systems gather dust. We need psychological safety - deep shifts in values, behaviors, and structure, not just mechanics.

If I may add, please try to avoid speaking of anyone "being at fault". Try to help people understand that it is no use trying to blame someone, or figuring out "whose fault" is was, but instead to look for deeper, often systemic, causes, and how to avoid them in the future.
The premise being, it probably made sense to the people at the time, to do what they were doing. Rather ask, "why" were they led to believe it was the right thing to do. Pilots and air traffic controllers are very rarely reckless or stupid or suicidal.

Lawyers and unions representing rail workers say there is a clear pattern across the industry of railroads retaliating against workers who report safety violations or injuries on the job. Often these workers run afoul of managers who don’t want to jeopardize their bonuses. The examples of retaliation discourage other workers from speaking up, out of fear they also could be disciplined or fired. The railroads say they are trying to eliminate the practice by prohibiting retaliation and creating multiple ways for workers to report safety concerns. The number of cases are down over the past five years, but government statistics show whistleblower cases continue to be filed every year.
Had a few rare hours of no meetings, and as it was tipping down outside decided to crack on with finishing this book.

Would recommend for those interested in system safety, healthcare safety and safety science.
