Today is National HIV and Aging Awareness Day (NHAAD). In 2021, there were 441,259 people aged 55 and older living with HIV, representing 41% of the U.S. population living with HIV. We must pay attention to the increasing number of people living long, full lives with HIV and their unique health and social needs, as well as the challenges of HIV prevention, testing, treatment, and care in older adults.

The need for a health equity framework in next-generation pre-exposure prophylaxis implementation

Pre-exposure prophylaxis (PrEP) is highly effective for prevention of HIV transmission and plays an important role in efforts to end the HIV epidemic within the next decade. However, disparities in access to PrEP might be fuelling disparities in the burden of HIV in the USA. The advent of next-generation PrEP formulations that do not involve daily regimens (eg, long-acting cabotegravir) holds potential to facilitate medication adherence, but if the roll-out of these formulations does not consider disparities in access, HIV disparities might be further widened.

Additional data from a mixed-methods study by Emory investigators exploring preferences for clinic communications regarding appointment reminders and peer outreach among people with HIV who were out of care. Findings showed a wide range of preferences and highlight the need for offering multiple options to promote re-engagement in care.

Data from the CDC's Medical Monitoring Project: HIV Care Continuum outcomes among cisgender women with HIV did not show any meaningful improvement from 2015 - 2019. In fact, the percentage whose HIV had ever been classified as Stage 3 increased in 2019.

In order to end HIV as an epidemic in the U.S. we need to address this effectively. To do that, we must deal with social and structural determinants of health.
