A pandemia foi marcada por um cenário caótico sob a gestão do ex-presidente Jair Bolsonaro (PL). O negacionismo, a defesa da "gripezinha" e o combate ao uso de máscaras e vacinas transformaram uma crise sanitária global em uma crise nacional. O descaso com a vida era visível, a começar pela instabilidade no Ministério da Saúde, que viu quatro ministros em apenas um ano — justamente no período mais delicado do avanço da doença. Enquanto o governo ignorava ofertas de imunizantes e promovia tratamentos sem eficácia, estados e municípios lutavam sozinhos. O resultado foi devastador: o Brasil atingiu a marca de mais de 700.000 vidas perdidas durante a pandemia, muitas delas que poderiam ter sido poupadas com uma condução baseada na ciência. Somente agora, com as investigações autorizadas pelo STF no ano passado, o país começa a ver passos concretos em direção à responsabilização. 'found that people who had contracted COVID-19 before vaccination had a weaker immune response to the vaccine than people who were never infected. Although vaccination does provide additional protection against SARS-CoV-2 infection and reinfection, these findings suggest that COVID-19 can cause long-term damage to the immune system and make someone less able to fight off reinfection.'

Nirmatrelvir Resistance in an Immunocompromised Patient with Persistent Coronavirus Disease 2019

Although the coronavirus disease 2019 (COVID-19) pandemic is coming to an end, it still poses a threat to the immunocompromised and others with underlying diseases. Especially in cases of persistent COVID-19, new mutations conferring resistance to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) therapies have considerable clinical implications. We present a patient who independently acquired a T21I mutation in the 3CL protease after nirmatrelvir exposure. The T21I mutation in the 3CL protease is one of the most frequent mutations responsible for nirmatrelvir resistance. However, limited reports exist on actual cases of SARS-CoV-2 with T21I and other mutations in the 3CL protease. The patient, a 55 year-old male, had COVID-19 during chemotherapy for multiple myeloma. He was treated with nirmatrelvir early in the course of the disease but relapsed, and SARS-CoV-2 with a T21I mutation in the 3CL protease was detected in nasopharyngeal swab fluid. The patient had temporary respiratory failure but later recovered well. During treatment with remdesivir and dexamethasone, viruses with the T21I mutation in the 3CL protease showed a decreasing trend during disease progression while increasing during improvement. The impact of drug-resistant SARS-CoV-2 on the clinical course, including its severity, remains unknown. Our study is important for examining the clinical impact of nirmatrelvir resistance in COVID-19.

Geschlechtsunterschiede bei Long-COVID-Symptomen und Immunprofilen

DMZ – FORSCHUNG ¦ Anton Aeberhard ¦ Eine kürzlich durchgeführte Studie untersuchte die Geschlechtsunterschiede bei Langzeitfolgen von COVID-19, bekannt als Long COVID (LC). Es wurde festgestellt, dass Frauen signifikant häufiger als Männer von LC betroffen sind, nachdem sie sich mit dem SARS-CoV-2-Virus infiziert hatten. Die Studie untersuchte, ob die immunologischen Merkmale von LC zwischen den Geschlechtern unterschiedlich sind und ob diese Unterschiede die unterschiedlichen Symptome von LC erklären können. Die Forscher führten eine querschnittliche Studie mit 165 Personen durch, von denen einige LC hatten und andere nicht. Sie untersuchten ein breites Spektrum von immunologischen und endokrinen Parametern und fanden heraus, dass Frauen und Männer mit LC unterschiedliche Symptome erlebten und verschiedene Immunprofile aufwiesen. Frauen hatten eine höhere Symptomlast und zeigten eine erhöhte Reaktivität gegenüber bestimmten Herpesviren im Vergleich zu ihren männlichen Gegenstücken. Maschinelles Lernen half dabei, die unterschiedlichen Immunmerkmale zu identifizieren, die LC bei Frauen und Männern charakterisieren. Männern mit LC wurden verringerte Häufigkeiten bestimmter Immunzellen und erhöhte Plasmazytokine festgestellt, während Frauen mit LC ein erhöhtes Vorkommen erschöpfter T-Zellen und zytokinproduzierender T-Zellen sowie niedrigere Testosteronspiegel aufwiesen. Besonders interessant war die Beobachtung, dass niedrigere Testosteronspiegel mit einer höheren Symptomlast bei LC-Patienten verbunden waren, unabhängig vom Geschlecht. Dies legt nahe, dass Testosteron eine wichtige Rolle bei der Immunantwort auf LC spielen könnte. Die Studie ergab auch, dass Frauen mit LC eine erhöhte Reaktivität gegenüber bestimmten Herpesviren wie EBV aufwiesen, was mit Autoimmunerkrankungen wie Multipler Sklerose in Verbindung gebracht wird. Es wurde festgestellt, dass Testosteron eine wichtige Rolle bei der Regulierung der Immunantwort auf diese Viren spielen könnte. Die Forscher betonen, dass ihre Ergebnisse nicht nur die geschlechtsspezifischen Unterschiede bei LC verdeutlichen, sondern auch mögliche Therapien aufzeigen könnten, die die Schwere der Symptome von LC durch hormonelle Behandlungen verringern könnten. Die Studie trägt dazu bei, ein besseres Verständnis für die geschlechtsspezifischen Unterschiede bei LC zu entwickeln und mögliche therapeutische Ansätze aufzuzeigen, die darauf abzielen, die Immunantwort bei LC zu modulieren. > Zur Studie

Wir haben soeben einige #COVI19 #Antigentests entsorgt, weil die Flüssigkeit in den Röhrchen gelblich und auf weniger als die Hälfte geschrumpft war. Das Ablaufdatum liegt weit im der Zukunft (Jänner 2024). Der Hersteller ist #Newgene Bioengineering.
