Is het een vlijmscherpe onderhandeling of chantage bij uitstek? Zambia, een land waar 11 procent van de bevolking lijdt aan de levensbedreigende infectieziekte hiv, krijgt al jaren vanuit de Verenigde Staten gratis medicatie tegen het aidsvirus. Maar de Amerikaanse minister van Buitenlandse Zaken Marco Rubio dreigt die hulp stop te zetten als Zambia niet instemt met een nieuwe, Amerikaanse gezondheidsdeal.

Wat er op tafel ligt, leidt tot grote woede in het Zuid-Afrikaanse land. De VS willen de komende vijf jaar nog 1 miljard dollar bijdragen aan de Zambiaanse gezondheidszorg, maar alleen als de Amerikanen de komende 25 jaar toegang krijgen tot Zambiaanse gezondheidsdata én Amerikaanse mijnbouwbedrijven kobalt en koper mogen delven in het land. De Zambianen hebben tot woensdag 1 april de tijd gekregen om te beslissen.

Landen wier hele zorgsysteem op die hulp was gebouwd, werden hard geraakt. Een jaar later is het financiële gat dat toen viel nog altijd niet opgevuld. Sterker: het probleem is alleen maar groter geworden, omdat ook Europese landen hun ontwikkelingshulp hebben teruggeschroefd.
